Treatment Coordination

Treatment Coordination is available to all clientele accessing services through The Phoenix Network. Treatment coordination begins at the point of referral and continues through to the initial admission process wherein treatment needs are identified and organized for the client. In addition, the role of the Treatment Coordinator extends to managing the discharge needs of the client upon completion of therapies. Primary contact with the client, family members, referral and funding sources, medical and professional practitioners occurs with the Treatment Coordinator.
